PHNs connect local health services — including My Aged Care, GPs and hospitals — and many regions offer Care Finder support.
Step 1 — Open the official PHN finder

If the map opens, zoom to your area or search by your suburb/postcode.
Step 2 — Prepare a quick message
Step 3 — What to ask for
- “Can you direct me to a Care Finder provider in my area?”
- “What aged-care navigation or social work support is available locally?”
- “How can our GP better link with local health and aged care services?”
